We at Valet Parking Authority pride ourselves in striving to be the most responsible and safest valet company in the Chicagoland area.  Unfortunately, sometimes accidents happen which is why we at VPA try our best to walk you through the claim process as stress free as possible.  

In the unfortunate case that an incident occurs, we ask that you please ask to speak to the VALET MANAGER.  If you are off-site, please RETURN  to the location where the incident occurred and again, ask to speak with the valet manager.  The valet manager will then ask you to fill out a claim form.   

A claim form is also known as an incident report.  Such forms must be filled out by the onsite manager immediately followed by a customer complaint of damage to his/her vehicle.  No claim is considered without a completed form.  

This form is to be used when:
  • Reporting Personal Theft
  • Reporting Stolen Vehicle
  • Reporting Personal Injuries
  • Reporting Damage to Vehicles
  • Other Incidents Requiring a Claim Form

Regardless if we are liable or not we require a claim form.

Filing a claim form does not mean that Valet PArking Authority, Ltd is liable for any claims or incident.  It is a report that is filled out by a valet manager of Valet Parking Authority for the customer and is turned in to the Claims Department for review.  

VPA personnel ARE NOT AUTHORIZED to settle claims at the location.  ONLY  the Claims Department has authority to settle claims.

Once the claim form is completed, our VPA manager will hand you a carbon copy of the form.  The VPA managers are trained to take photos of the incident.  Please feel free to take photos as they are welcomed.  

The photos and the claim form will then be sent to the claims department at 2342 N. Damen Chicago, IL 60640.  Claims are received and investigated by our General Manager, Manuel Feliciano.


If you have not received a call by Tuesday or Wednesday after the incident, please feel free to contact us during our office hours at (312) 867-4911 so that your claim may be processed as soon as possible.
